Event Request Form for Student Organization

  • Please Read:

    Please submit this form when you need to reserve rooms or services from the Events Office for your event. Please read carefully the timeline of the process below, and submit your request promptly. 1) Submit this form to request pre-approval of your event at least 1 month before your event date. 2) You will be notified when Student Life has approved your event request with the Events Office. 3) The deadline for you to make any changes to your event is 2 weeks before the event date. 4) Any changes after this date incurs a $100 fine.

    Your choice of up to two different sandwiches options per event. Sandwiches feature fresh cut meats, cheeses, and garden fresh vegetables. Soda assortment included. Served with Kettle chips, apple and a cookie. $10.95 each.
