Maintenance Request Form
Please fill out this form for any maintenance related issues in your dormitory or apartment. We are always happy to serve!
Intent to Vacate Form
This form is a required formal notice if you are planning to move out of your apartment. If you are planning to move off campus from an SBTS apartment, please fill out the following form at least 30 days in advance.
Apartment Checkout Form
This form is required to be filled out at the time that you turn in your apartment or dormitory keys, which you can do at Campus Police or the Housing Office.
Housing Visitor Form
This form is required for dormitory students only. Please fill out this form for any overnight visitor at least 24 hours in advance.
